Physical Examination and Medical Release Forms

Please make sure all the forms included are filled out by you or a doctor.  Also keep in mind that we are not part of the IHSAA and therefore we have some different eligibility rules.  However, some of our rules are the same or similar and for that reason we need all the forms filled out properly in order for the student athlete to participate in Central sports. 

Each athlete must receive a new sports physical for each school year.  Please make your appointments early and have the forms ready to turn in at Central's registration in August.  Thanks.
